The strange situation - could not upload document (SPIP 2.1.12+)

Greeting,
during last time we often see the strange matter with SPIP ( we use
2.12, 2.13, 2.14)
we could not upload the document to article ( rubrique), but could add a logo.

- All permissions are correct (777 IMG, 777 IMG/*/)
- The setting is correct (allow document to article etc)
- If we uploading the file to /tmp/upload the SPIP doesn't see it
- no record in database ( spip_documents*) created
- Any logo uploading well

- if we copy the SPIP ( dump + squelettes + images) to another hosting
we are keeping the matter
- Upgrading 2.1.12->2.1.13->2.1.14 doesn't help

- if we upload a new SPIP install we could create an article and add
document to it, but when we
restore from dump.xml the matters appear again

We get this matters often ( 1 of 3 SPIP installation), and see the
matters at least at 3 different
hosting company ( our developing server + 2 big industrial hosting
where now normally works at least 40 other SPIP website).

Is any idea or solutions?

I've found the solution

The matters is the blank table spip_types_documents in the database.
I just copied the table from another website and everything works fine.

I have lots of the same problems.. but in spip 3.0..
In 3.0 I have to manually make new folders in the local/cache-vignette folder
Spip creates folders there that have the same size as the picture.. eg.. L500xH300 ( lenght x hight ) ( i guess )
I keep having problems with many of the new folders that have ( according to my server ) different UID ( user ID ) … when I delete ( one at the time ) folder/s and make a new one and chmod them 0777 everything is perfect.
In earlier ( 2.09 ) versions the only problems I had waw in the tmp folder, where I had to delete several folders ( wrong/diffente UID ) make new ones with the same name.. chmod 0777 and everyting was fine again..

my 3.0 site is working fine now.. only problem is that i had to make more than 2000 folders manually ( the eg.. L500xH300 folders in the local/cache-vignette folder ) Again when you know the problem.. you can solve it !
