The documentation of the form plugin is here :
you can also insert your form in specific templates with
#CACHE{0}
[(#MODELE{form}{id_form=1})]
and include it in article-10.html, or call it with such rl :
spip.php?page=myForm
More exactly the documentation is a little old-dated because it was
for an old version of the plugin. But it's the more complete I know.

Hello
I'm quite a beginner with SPIP and my English is not as good as it
used to be, but I'm gonna try to help.
I took some time to understand how works this plugin, too. You have
to prepare your form first with the integrated tool and then insert
it in an article, like in Image 1.
Your form may represent the sole data of the article. My example is
here : http://www.chefprestige.com/Demande-de-devis
I think you have a misunderstanding and use "form" where you should
use "table", no ?
